• Match Report - FILLED OUT (Link above will be activated soon) Please print on cardstock (heavy paper)in Landscape Mode.
• Laminated Player Cards - 2007/2008 season - for every player, coach & team manager.
• Current Player Registration Form (Medical Releases) & provide a copy of Birth Certificate in case of a problem
• Hotel Information if staying the night
• Emergency Contact cell phone #’s for both your coach & manager
• Travel papers - if needed

All affiliated teams outside So-Cal are guaranteed entry; however, no refunds will be made after immediate receipt of applications. This policy allows teams to make airline reservations, process travel papers and reserve hotels.
